They can/are suppose to run a check against your VIN# to confirm your truck is part of the TSB. I would print out the TSB and take it in and have them double check.

It shouldn't. Go by an auto parts store and see if any trouble codes are stored in the system. They can clear them if so.. Major auto parts store do this for free.
I haven't, but it looks like something I wouldn't trust on my $50K truck. Also, you can't even flash an EcoDiesel via the OBD port. So I would say it's just a fooler anyways.
Go with something tried and true. GDE, PPEI, etc..

Sound like a streak of bad luck as hit you. The electronic throttle control code is random, and I'm not sure I've seen one fix or solution yet for it. Some have the problem, and some do not.
For your other issues, all new to me personally. Let's hope the dealer can get...

Craig, the only exhaust that works with emissions intact would be a DPF back exhaust, and those are for looks only. You won't have any sound differences.
To run a full aftermarket exhaust , and to hear a sound difference, you would have to buy "off road tuning" that disables the emissions...
